Shut-Down Reporting

Use Shut-Down on the Single Site Reporting page to review shut-down checklists for your site: scheduled and unscheduled completions, assigned tasks, and issues found when plant or a process is taken out of service.

The report page title is Shut Down. For all 12 report types, see Site Wide Reporting.

Note: Single Site Reporting is available to Managers and above, including Line Managers.

Open the Shut-Down report

  1. Log in to the ComplianceTrackr desktop portal.
  2. In the left menu, open Reporting Dashboard.
  3. Select Single Site Reporting.
  4. On the Single Site Reports screen, select Shut-Down.

Summary cards

The top of the page has eight cards. Dates display as DD-MM-YYYY.

  • Forms Scheduled — monthly, weekly, and daily totals. Select View Schedule.
  • Un-Scheduled Forms Complete — use Select Date range.
  • Tasks Assigned — labelled Statistic. Use Select Assignee, then View Tasks.
  • Tasks Outstanding — labelled Percentage. Use Select Assignee.
  • Scheduled Forms Complete — use the date range, then View Completed.
  • Incomplete Schedule Forms — use the date range, then View Incompleted or Unattempted Forms.
  • Completed Tasks — labelled Statistic. Use Select Assignee, then View Tasks.
  • Number Forms Completed By — use Select Date range and Staff Name.

If a card shows 0, change the date range or assignee. Shut-down forms are often used less often than daily pre-start forms, so empty cards are common.

Scroll down to Shut-down Form Submitted.

Turn on Show Only Inprogress Form? if you only want forms that are still in progress.

From this table you can open a completed form, send a reminder for an incomplete form, or open Reminder Activity logs. A reminder can be sent once per day.

Shut-down issues

Issues raised on shut-down forms are grouped by status:

  • Shut-down Issue Identification - Pending
  • Shut-down Issue Identification - In Progress
  • Shut-down Issue Identification - Closed
  • Shut-down Issue Identification - Open
  • Shut-down Issue Without Controls

If a table says “No completed forms found for the specified criteria” or “No data available in table”, there are no issues in that status.
