Incident Reporting

Use Incident Reporting on the Single Site Reporting page to review incident forms for your site: completions, open and closed issues, incident type, locations, injury types, contributing factors, severity, and medical issues.

For all 12 report types, see Site Wide Reporting.

Note: Single Site Reporting is available to Managers and above, including Line Managers.

Open the Incident report

  1. Log in to the ComplianceTrackr desktop portal.
  2. In the left menu, open Reporting Dashboard.
  3. Select Single Site Reporting.
  4. On the Single Site Reports screen, select Incident Reporting.

Summary cards

The top of the page has four cards.

Incident Complete Form

Labelled Average and titled Incident Complete Form. It shows how many incident forms were completed in the selected date range.

Use Select Date range to change the period. Dates display as DD-MM-YYYY. The page opens on today’s date.

Issues Identified

Labelled Numbers. It shows:

  • Open issue
  • Closed issue

Each count has its own date range: Open Issues Date Range and Closed Issues Date Range.

Incident Type

Labelled Numbers. Use Select Incident to choose an incident category set up for the site, such as Workers, Visitor, RSA, RSG, HR, Notifiable, Security, or Other.

The large number is how many completed incident forms match that category.

Open Actions

Labelled Percentage. Use Select Incident again. The percentage is how many actions in that category are still open (not closed).

If no actions exist, this card shows 0%.

Overview charts

Incident Reporting includes five yearly charts. Each chart has its own Select Year dropdown and opens on the current year. Use the minus icon on a chart heading to collapse that section.

Incident locations

Heading: Report Based on Incident Locations. Label: Overview Incident Locations.

Chart title: Incident Reporting based on Incident Location. Vertical axis: Incident Locations Level By Numbers.

Injury types

Heading: Report Based on Injury Types. Label: Overview Injury Type.

Chart title: Injury Reporting based on Injury Type. Vertical axis: Injury Type Level By Numbers.

Incident factors

Heading: Report Based on Incident Factors. Label: Overview Incident Factors.

Chart title: Incident Reporting based on Incident Factors. Vertical axis: Incident Factors Level By Numbers.

Incident severity

Heading: Report Based on Incident Severity. Label: Overview Incident Severity.

Chart title: Incident Reporting based on Severity. Vertical axis: Incident Severity Level By Numbers.

Typical severity ratings include Negligible, Marginal, Critical, and Catastrophic, where those options are used on the form.

Medical issues

Heading: Report Based on Medical Issue. Label: Overview Medical Issue.

Chart title: Incident Reporting based on Medical Issue. Vertical axis: Incident Medical Level By Numbers.

All five charts show months from January to December. Location, injury, factor, and medical series come from the answers recorded on incident forms for that year.

Incident forms submitted

Scroll down to Incident Form Submitted for each completed incident form.

Turn on Show Only Inprogress Form? if you only want forms that are still in progress.

From this table you can:

  • open a completed form, which shows form type, submitted by, submission time, and whether the form is in progress
  • send a reminder for an incomplete form — the user receives an email and an in-app notification
  • open Reminder Activity logs to see who sent a reminder, who received it, and when

A reminder can be sent once per day. If the reminder button is unavailable, it can be used again tomorrow.

Incident issues

Issues raised on incident forms are grouped by status:

  • Incident Issue Identification - Pending
  • Incident Issue Identification - In Progress
  • Incident Issue Identification - Closed
  • Incident Issue Identification - Open
  • Incident Issue Without Controls

Open an issue’s action details to see controls, the action taken, who took it, who closed the ticket, accept or reject notes, status, and times.

If a table says “No data available in table”, there are no issues in that status for the current filters.
